What the 2023 ATI PN/VN Fundamentals proctored exam covers

ATI organizes Fundamentals around five content domains, scoped to what a PN/VN performs and reports. Our 70-question practice exam distributes questions across them in a similar proportion to what you'll see on test day. The 2023 series is predominantly classic four-option multiple choice with a smaller number of select-all-that-apply items, and our 2023 exam matches that mix.

1

Foundations of Practice & Clinical Judgment

PN/VN Fundamentals questions test how you apply the nursing process — assessment, prioritization, and action — within the PN/VN scope. Expect ABC-style priority questions (airway before breathing before circulation) and items that test what a PN/VN reports to the RN versus what they can act on independently. Delegation questions here run in the opposite direction from the RN exam: instead of asking what you can delegate, they ask what a PN/VN can safely perform and what must be referred up to the RN or provider. Know that boundary cold — it's the single biggest scoring lever on this exam.

2

Safety & Infection Control

Standard and transmission-based precautions, PPE donning/doffing order, fall-risk identification, fire safety (R.A.C.E./P.A.S.S.), and medication/equipment safety make up a large share of the exam, and this content is nearly identical at the PN/VN and RN scope — a fall precaution or a PPE sequence doesn't change based on who's performing it. These questions are procedural: there's one correct sequence or one correct precaution level, so memorizing the exact order (gloves come off first) pays off directly.

3

Health Assessment & Documentation

You'll be tested on normal vs. abnormal findings across systems (vital signs, pulses, skin, respiratory effort) and how to document objectively — but PN/VN items lean harder on data collection and reporting than on independent assessment. A PN/VN commonly collects and documents findings, then reports abnormal results to the RN for further assessment and clinical judgment. Practice recognizing which findings are normal, which need reporting, and which are outside PN/VN scope to interpret alone.

4

Physiologic Needs

This domain covers hygiene, mobility, positioning, elimination, sleep, nutrition, hydration, and skin integrity — largely hands-on care that sits squarely within PN/VN scope. Expect lab-value questions, NPO and intake/output scenarios, and basic care-skill questions. These are usually knowledge-recall items, which makes them some of the most efficient points to lock in with focused review.

5

Psychosocial Needs

Culturally competent care, therapeutic communication, coping and loss, and client rights and confidentiality fall here. The trap in this domain is the same at every scope: picking the answer that sounds kind rather than the one that's actually client-centered. A PN/VN's role is usually to gather that information and communicate it clearly, then loop in the RN for care planning that requires independent judgment.

Sample questions from the 2023 PN/VN Fundamentals exam

Three questions written to the PN/VN Fundamentals scope, in the exact style of the 70-question 2023 exam, with the correct answer and rationale shown. The full PDF puts the answer key in a separate section so you can sit the exam untimed or timed first.
The RN asks an LPN/LVN to assist with updating a client's plan of care. Which action is within the LPN/LVN's scope?
A. Writing a new nursing diagnosis for the client
B. Contributing observations from ongoing care to the RN's care-planning process
C. Approving a change to the client's plan of care
D. Discontinuing an intervention that is no longer effective

Rationale: LPN/VNs contribute observations and data to the RN, who retains responsibility for developing, approving, and revising the plan of care.

An LPN/LVN is caring for a client who has a provider's order for a vest restraint. Which action should the LPN/LVN take?
A. Apply the restraint and leave it in place until the end of the shift
B. Check the restraint and the client's skin at least every two hours
C. Remove the restraint only if the family requests it
D. Tighten the restraint if the client attempts to remove it

Rationale: Restrained clients must be checked periodically, commonly at least every two hours or per facility policy, to assess circulation, skin integrity, and the continued need for the restraint.

An LPN/LVN auscultates new bilateral crackles in a client's lungs that were not present on the previous shift's assessment. What should the LPN/LVN do first?
A. Document the finding and continue with routine care
B. Reposition the client and recheck in two hours
C. Report the new finding to the RN
D. Encourage the client to use an incentive spirometer

Rationale: A new, acute change in assessment findings must be reported to the RN for further evaluation. The LPN/LVN should not independently manage a newly identified abnormal finding.

ATI PN/VN Fundamentals 2023 exam FAQ

1

How is the 2023 exam different from the 2026 exam?

The 2023 series uses the earlier Fundamentals blueprint and is mostly classic multiple choice plus select-all-that-apply. Programs that moved to the newer series should use the 2026 exam, which adds NGN-style case studies and ordering items.

2

How many questions are on the ATI PN/VN Fundamentals 2023 proctored exam?

ATI's official exam typically runs a similar range of scored items as the RN version, plus a small number of unscored pretest items, so a 70-question practice exam gives you a realistic full-length run. Your program sets the exact count and time limit — check your syllabus.

3

Which version does my school use?

Ask your instructor or check the assessment name inside your ATI account — the series year is shown on the assessment card. Not sure yet? The PN Fundamentals overview lists all three Fundamentals products so you can compare.

4

Is this the actual ATI 2023 exam?

No. This is an independent, original practice exam written to cover the same content areas and question style as ATI's 2023 PN/VN Fundamentals content-mastery exam. It is not affiliated with, endorsed by, or a reproduction of Assessment Technologies Institute's proprietary exam content.

5

Should I also get the 70-question Study Guide?

The ATI PN Fundamentals Proctored Exam Study Guide (70 questions with rationales) is the broad content review; this 70-question exam is the timed dress rehearsal. Most students do the study guide first, then sit the exam for their version a few days before test day.
